Fees
Mark’s standard fee is $150 per appointment of individuals and $200 for couples (52 minutes). Group therapy is by sliding fee scale, depending on income and ability to pay: $25-$75).
Mark is in-network with Blue Cross and Anthem health insurances, Medicare, Green Mountain Care, and Medicaid. Many health insurance companies offer out-of-network benefits, and Mark can issue a "super receipt" that you can submit to your insurance company for reimbursement. It is your responsibility to check with your insurance company to see if they offer out-of-network benefits before making an appointment. Payment is due at each appointment.
